ACLS Drugs

It is important to know which drug to use during a code. We wanted to take a few minutes and review some the ACLS drugs used in the ACLS algorithms.

Atropine:

  • is the first drug of choice for symptomatic bradycardia
  • it is classified as an anticholinergic drug and increases the firing of the SA node by blocking the action of the vagus nerve on the heart resulting in an increased heart rate
  • dose in the bradycardia algorithm is 0.5 mg IV push and may repeat up to a total dose of 3 mg

Amiodarone:

  • is a potent antiarrhythmic agent that is used to treat ventricular arrhythmias and atrial fibrillation
  • when you have ventricular fibrillation / ventricular tachycardia that is refractory to shock (meaning that after 2 or 3 consecutive defibrillations and the rhythm remains unchanged) you can consider amiodarone or lidocaine if amiodarone is not available
  • it is used restore normal heart rhythm and maintain a regular, steady heartbeat
  • first dose for cardiac arrest is 300 mg and repeat with 150 mg every 3 to 5 minutes for maximum dose of 450 mg

Epinephrine:

  • is the primary drug administered during CPR to reverse cardiac arrest
  • it increases arterial blood pressure and coronary perfusion during CPR
  • acts quickly to improve breathing, stimulate the heart and raise a dropping blood pressure
  • dose is 1 mg every 3 to 5 minutes

Adenosine:

  • is an antiarrhythmic that works to treat irregular heartbeat such as tachycardia with a pulse
  • it slows the electrical conduction in the heart, slowing heart rate or normalizing heart rhythm
  • first dose of adenosine should be 6 mg pushed rapidly over 1 to 3 seconds followed by a 20 mL normal saline bolus
  • if the first dose does not convert out of SVT within 1 to 2 minutes, a second 12 mg dose can be given in a similar fashion
